Duration of your hospital stay

We start to talk about discharge with you as early as possible and we will inform you as soon as possible of the date and destination of your discharge.  Sometimes our assessments show that you may need additional support at home, or that, in some cases, it may no longer be possible for you to manage at home, in which case moving to residential or nursing care may be necessary.

Our multidisciplinary team will attempt to get you home or to a more suitable alternative as soon as you are well enough. 

If necessary and appropriate a rehab review meeting will be arranged to discuss on-going needs.  Your family or loved ones will be invited to attend this with you. 

The team are committed to working with many other agencies such as other hospitals, adult social care and community based health professionals. However, cross border discharges and transfers can be more complex.
