New patient portal means health and wellbeing information is just a few clicks away

Patients using Wye Valley NHS Trust’s services are benefiting from a new on-line platform that allows them to access information about their healthcare from their mobile phones, laptops and tablets.

The new patient portal went live during the summer and is proving a hit with around 2,000patients already signed up.

The patient portal means that patients can:

  • Review their outpatient appointment details
  • Receive future appointment and clinical letters along with associated leaflets Access contact details for each service
  • Create “my circle” – the ability to give permission for family/friends to view information through the portal

“The patient portal – called My Health and Wellbeing - is available to all our patients, aged 16-year-of-age and older, and provides convenient access to health information. This includes access to important letters which would otherwise have been put in the post, which could have taken days to arrive in the past,” said Jane Ives, the Trust’s Managing Director.

“The portal is easy to use and brings together health and wellbeing information in one easy to access place,” added Jane.

The portal has been developed for NHS patients across Herefordshire and Worcestershire – the Herefordshire version is the first to go live and is set to become the go-to portal for all patient related information, letters and leaflets. The portal integrates with the NHS app.

Signing up to the portal is optional for patients, but it makes accessing important clinical information much easier as the portal will run on smartphones, computers and tablet devices.

The portal includes helpful resources, including health trackers and a health video library with more than 1,000 patient information videos, to help patients find resources to support their care.

“Importantly, patients can also create their own support network by inviting family, friends, or carers to join their ‘circle of care’, so they can share information with those they chose to support their care,” added Jane.
