We start to talk about discharge with you as early as possible and we will inform you as soon as possible of the date and destination of your discharge.  Sometimes our assessments show that you may need additional support at home, or that, in some cases, it may no longer be possible for you to manage at home, in which case moving to residential or nursing care may be necessary.
