About the Service School Nurses are public health nurses working in the community, they are registered nurses with an additional public health nursing qualification. School Nurses have specialist knowledge and skills to enable them to work with children and young people aged from 5-19. We start to talk about discharge with you as early as possible and we will inform you as soon as possible of the date and destination of your discharge.  Sometimes our assessments show that you may need additional support at home, or that, in some cases, it may no longer be possible for you to manage at home, in which case moving to residential or nursing care may be necessary.
