Community Paediatrics
We are a small specialised team providing a Consultant-led service dedicated to improving the health and well-being of children, young people and their families.
We deliver a high quality service to families with children suffering from developmental disorders and disabilities, children in need of protection and children in care. We support children with life limiting conditions within the family.
We can see you in the Child Development Centre, a community hospital, children's centre or special school. Our doctors will see you in the most appropriate place depending upon your address and your child's condition. Home visits are available in special circumstances. You can choose to use our service but will need a referral to us from your GP, health visitor, school nurse or social worker.
We will work alongside the other health and social care providers to provide the highest standard of care for you particularly if you require complex packages of care and support. You and your family will be actively involved in all decisions about your care by our consultant-led team.

Autism assessments - documents
The questionnaires on this page are used to gather information from parents, carers, and educational settings about a child who may be referred for an autism assessment. They help to build a comprehensive picture of the child’s strengths, needs, and everyday functioning across different environments.
If you are a parent / carer, please download and complete:

Please complete all questionnaires as fully and honestly as possible, based on the child’s typical behaviour rather than isolated or recent events.
All questionnaires must be sent back to school and sent in alongside a referral for the child.
